We require a complete medical history for each patient entering our practice. We require a full copy of your child's medical records, including vaccine records before we schedule your first visit. We ask that you arrive 30 minutes prior to your first appointment.
To help provide timely service to our patients, we prefer to schedule all visits. If you arrive late for your appointment, we will try to work you back into the schedule, or perhaps offer an appointment to you with another provider. Out of courtesy to our other patients, we may be forced to ask you to reschedule your appointment.
If you are unable to keep an appointment, please notify us 24 in advance. If you fail to keep an appointment, you will receive a telephone call and a letter reminding you that you missed your appointment and remind you to reschedule. If you miss three appointments without adequate notice, we will reluctantly ask you to choose another doctor’s office for your child’s health care.
If a new patient misses an initial visit without 24-hour advance notice, the appointment will not be rescheduled, nor will the patient be admitted to the practice.
We do not accept walk-ins at either of our offices. All visits require an appointment.
Patients who walk in with a non-emergency condition will be triaged by a nurse and asked to schedule the next available sick appointment.
